Exceptional Care - Understanding the Circumcision Procedure

World Class Providers

Dr. Tomlinson wrote the surgical chapters for the World Health Organization’s manual on Early Infant Male Circumcision. He has been a consultant to the WHO, NIH, Gates Foundation, and has trained health care providers in newborn male circumcision all over the World. He led the international team of physicians and engineers that developed the Minimally Invasive AccuCirc® Technology. Dr Tomlinson is recognized as a global expert in newborn male circumcision and has dedicated his career to improving newborn male circumcision care and protecting newborns from injury during the procedure.

Timming

There is no reason to rush this procedure. Mom and baby need time to recover. The first 24 hours of life should be spent bonding, not undergoing a procedure. Our Center of Excellence allows parents to choose a day and time outside the busy, stressful hospital setting, allowing mom and baby to recover before the procedure. The procedure can be scheduled anytime in the first 28 days of life.

Pain Control

Comfort is paramount during procedures at our facility. We provide local anesthesia to minimize any pain your newborn may feel. Before administering anesthesia, we offer the option of breastfeeding or the use of breast milk to soothe your baby. Additionally, we can provide sucrose water as an alternative to help ease discomfort, ensuring a comfortable and stress-free experience for both you and your newborn.

Pen Mark

Pediatric urologists have been advocating for the use of a surgical pen to mark the site of the incision to avoid errors in removing too much or too little tissue. In 1983, George Kaplan published a comprehensive review of the complications of newborn male circumcisions, and made the following recommendation: “Prevention of these errors is best achieved by marking the site of the corona on the skin surface prior to any incision.”

Despite these recommendations, most providers in the US do not use a surgical pen.

The use of a surgical pen is mandatory in our center.  This allows us to perform a very accurate procedure and reduces the risk of removing too much or not enough tissue.

Minimally Invasive

Male circumcision procedures traditionally involve an extra incision in the foreskin known as a dorsal slit for shield insertion, increasing the risk of penile injuries. However, our approach revolutionizes the process. By utilizing the minimally invasive AccuCirc® shield, we eliminate the need for a dorsal slit. This groundbreaking technique not only minimizes pain but also reduces the risk of injury, ensuring safer and more comfortable circumcisions for newborn males under our care.

Accurate

The Minimally Invasive AccuCirc® sets a new standard in male circumcision with its foreskin holder. This revolutionary improvement ensures precise tissue removal by applying gentle pressure to hold the foreskin in place. By aligning the foreskin accurately before incision, providers reduce the risk of tissue discrepancies. When combined with pen marking, this innovative feature minimizes the chances of over- or under-removal of tissue, guaranteeing optimal outcomes for newborn males undergoing circumcision.

Safer

The Minimally Invasive AccuCirc® uses a proprietary shield to protect the tip of the penis and is designed to eliminate mismatching parts. This ensures the correct size shield is used with the correct size clamp, no matter what, and guarantees that the tip of the penis is protected during the procedure. Other clamps do not have this feature. If the provider makes a simple mistake and mismatches the parts of the clamps, this can cause serious injuries to the newborn.

State-of-the-art Aftercare

At our center of excellence, we utilize the latest technology to ensure optimal outcomes, including the use of electrosurgery. While rare, bleeding may occur at the incision site during circumcision. Unlike traditional methods involving silver nitrate sticks, our state-of-the-art electrosurgical unit applies bipolar energy to effectively seal bleeding tissue. By embracing modern techniques, we mitigate complications and the potential for permanent scarring, offering simplified aftercare for parents and caregivers.

Improved Outcomes

Tissue adhesive has been a cornerstone in pediatric urology for infant male circumcision. One of the more common complications of newborn circumcision is adhesion formation. This occurs when one part of the wound heals to a part of the penis it is not supposed to. Tissue adhesive, also known as medical-grade surgical glue, acts as a sealant, ensuring optimal wound closure and healing. Additionally, it assists in minimizing bleeding and simplifying wound care for parents and caregivers, fostering a more comfortable and expedited recovery process for newborns.

No Mogen Clamp!

The Mogen clamp is associated with catastrophic injuries, including amputation of the tip of the penis. Please see a description of the conventional devices for an explanation of how this injury occurs every year in the US and around the World.

WE WILL NEVER USE A MOGAN CLAMP IN OUR CENTER OF EXCELLENCE! We promote education about this well described injury and the risks associated with the Mogen clamp. We encourage other providers and centers to eliminate the use of this potentially dangerous clamp.

Comprehensive Care

In Rhode Island and numerous areas across the United States, newborn circumcision is commonly performed by OB/GYN physicians in hospital settings, with limited follow-up care. This often leaves pediatricians and Family Medicine physicians to manage any complications post-procedure.

Recognizing the shortcomings of this approach, our center of excellence offers a different standard of care. We provide comprehensive support throughout the entire circumcision process, guiding you and your infant son from start to finish. With our attentive approach, we ensure that any issues are managed promptly, guaranteeing optimal outcomes and peace of mind for you and your newborn.

At our center, we prioritize your peace of mind by offering round-the-clock availability to address any questions or concerns you may have.
